For new event requests in the SP Lab, please complete the following form six weeks prior to your scheduled event. Someone will reach out to you to confirm the details of the request. If you need a case(s) created for your event, please let us know AT LEAST eight weeks prior to the event.
*Please note that due to multiple programs using the facility, dates/times requested may or may not be available. Rules and restrictions apply. Please check with the coordinator on scheduling availability. We will need a student roster to complete a rotation schedule for your event.
